Expanding Access to Healthcare for Returning Citizens

2020 - 2022

The Transitions Clinic Network (TCN) based out of Yale University and University of California San Francisco (UCSF,) is a national network of primary care clinics serving recently-released individuals returning home from prison. These individuals are more than twice as likely as the general population to be suffering from chronic mental and physical illness, compounded by economic, housing and racial disparities. TCN operates three Connecticut clinics, in New Haven, Bridgeport and Hartford, each serving up to two hundred new patients per year. This funding will help TCN open three additional Connecticut clinics in areas such as New London, Waterbury and Stamford to serve more patients and ensure healthy reentry.
