Planning Grant for the Transitions Clinic in New Haven

To convene and consult public officials and health leaders in Connecticut to identify ways to financially sustain this health clinic that addresses the growing health needs of men and women returning to the New Haven area from prison and to expand the model to other cities in the state with large populations of formerly incarcerated. The Transitions Clinic, located adjacent to the community health center at Yale University, is part of a national network of 13 such clinics. A key component of the project is the training and employment of formerly incarcerated men and women as community health workers who provide case management and support to patients. These services are not covered by medical insurance. The New Haven clinic annually serves between 125 and 150 clients.
