To help Friends create a network of partners and providers that will coordinate discharge planning and aftercare support for young people ages 16 to 21 returning to any of the five boroughs of New York City from the Rikers Island jail. The network will expedite the movement of youth through the court process and provide them with pre-release planning and post-release connections to age and developmentally appropriate community services to help improve their outcomes and reduce recidivism. These services will be funded by the city of New York after the initial planning process.
